By submitting this entry form, entrant (“User”) agrees to these Terms and Conditions for the Granite Orthopaedics PLLC February 2026 6-Session Laser Therapy Giveaway (“Giveaway”).

1. Sponsor: Granite Orthopaedics PLLC (“Provider”) reserves the right to modify these Terms at any time.

2. Entry Period: February 3, 2026 at 12:00 AM through February 28, 2026 at 11:59 PM MST.

3. Eligibility: One entry per person/email. Must be 18+, Arizona resident, and legally capable of consenting.

4. Prize: Six (6) laser therapy sessions. Must be used within two (2) months of first treatment. Certificate expires January 1, 2027. Redeemable only at Prescott Valley (2960 N Centre Ct, Prescott Valley, AZ 86314). No cash value. Non-transferable.

5. Medical Clearance: Winner must complete consultation and be medically approved by Provider. If not deemed a safe candidate, prize is forfeited without substitution.

6. Winner Selection: Winner notified by email from [email protected]. Confirmation required within 48 hours or prize will be redrawn.

7. Marketing Consent: Entry constitutes consent to receive email marketing communications. User may unsubscribe at any time.

8. Privacy & Publicity: Winner identity disclosed publicly only with written consent and signed media release. Provider may announce a winner has been selected once prize is claimed.

9. Patient Requirements: Winner agrees to provide accurate personal and medical information and complete required forms prior to treatment.
