Granite Orthopaedics PLLC's May 2025 6-Session Laser Therapy Treatment Giveaway
TERMS AND CONDITIONS:
By completing and entering the online form, "the User" (submitter of the online giveaway form including the name and email address subscriber on the form, patient, customer) agrees to all terms and conditions of "the Giveaway" (Granite Orthopaedics PLLC's May 2025 6-Session Laser Therapy Treatment Giveaway, Laser Therapy Giveaway) hosted by "the Provider" (Granite Orthopaedics PLLC, Granite Orthopaedics, Granite Orthopaedics Acquisitions Team, Staff), including but not limited to:
1. The Provider has the right to update the terms and conditions of the Giveaway at any time without notice.
2. The entry timeframe for the Giveaway is between May 1, 2025 12:00am through May 31, 2025 5:00pm.
3. One submission/entry valid per User, name and email address, for the entirety of the Giveaway entry timeframe.
4. The User agrees to being 18 years of age or older, resides in Arizona, and is of sound judgement to enter into the Giveaway.
5. The User agrees and understands all 6-Sessions of the Laser Therapy Treatment must be used within 2-months of the first treatment. The Giveaway certificate expires on January 1, 2026. The User may only fulfill all 6-sessions before January 1, 2026, at the Prescott Valley office located at 2960 N Centre Ct. Prescott Valley, AZ 86314.
6. The User understands, and agrees to, by providing their name and email address upon entry into the Giveaway, that the User subscribes to the Provider's monthly newsletter, received from [email protected], to receive marketing communications. The User may unsubscribe from the email list at any time.
7. The winning User will receive an announcement email from [email protected] stating their award of the Giveaway. The Provider requires a confirmation via returned email from the winning User within 48-hours of the sent timestamp of the announcement email. If winning User does not confirm receipt within 48-hours, the Provider will redraw a winning User. This method will continue until the Provider receives confirmation via returned email from a winning User and the Giveaway is deemed complete.
8. Due to HIPAA compliance, the Provider will only announce publicly the winning User by identity with approval from the winning User, and the Provider has a signed media release form from the winning User on file. The Provider will announce that a winning User has been drawn on the Provider's social media channels once the Giveaway's award has been claimed.
9. The User agrees to provide the Provider with all necessary, and accurate personal and medical information to complete and file a patient chart.
10. The User agrees to adhering to a Laser Therapy consultation with the Provider's chosen provider to determine User is a good candidate for Laser Therapy. Only if and when the chosen provider determines User is deemed a medically safe candidate, the User may fulfill the certificate of the Giveaway.
11. The User agrees, and understands, they have reviewed, completed and turned in the Provider's Laser Therapy forms and informational handouts, found at https://www.graniteortho.com/patient-forms/ or https://www.graniteortho.com/wp-content/uploads/2023/07/Lase..., before scheduling a consultation with the Provider's chosen provider.
