Terms & Agreement

By submitting this form, I agree to the following:

1. Participation & Conduct
   I understand that my child is enrolling in the Need 4 Speed Track & Field Summer Program. I agree to support my child in attending practices and competitions regularly and on time. I will encourage respectful behavior, good sportsmanship, and a positive attitude throughout the season.

2. Liability Waiver
   I acknowledge that participation in sports carries the risk of injury. I hereby release and hold harmless Need 4 Speed, its coaches, volunteers, and representatives from any liability arising from injury, illness, or accidents during practices, meets, or any program-related activities.

3. Medical Consent
   In the event of an emergency, I authorize the Need 4 Speed staff or coaches to seek appropriate medical attention for my child if I cannot be reached.

4. Photo & Video Release
   I grant permission for photos or videos of my child to be taken during program activities and used for promotional purposes, including social media, websites, and flyers.

5. Refund Policy
   I understand that registration fees are non-refundable and cover administrative costs, uniforms, meet fees, insurance, and program-related expenses.
